ROYALTY PARTICIPANT ADDRESS AND PAYEE CHANGE POLICY

The following procedures must be followed for all Provident Music Group royalty participant address and payee account change requests.
For ADDRESS CHANGES, the current payee must send a notarized copy of the form below which will represent a letter of direction in regards to the change you are requesting to our Legal and Business Affairs Department. This form must include the following information:
  1. The full name, address and when possible, all account numbers for which the person is a payee;
  2. A completed W-9 tax form (U.S. residents) or a W8-BEN form (Non-U.S. residents) for verification of your tax identification number or social security number.   3. The new address to which all future statements and payments should be sent.

For PAYEE CHANGE requests, the current payee must send a notarized copy of the form below which will represent a letter of direction in regards to the change you are requesting to our Legal and Business Affairs Department:
  1. The full name, address, and social security number or Federal Tax ID# of the current payee;
  2. The full name, address, and social security number or Federal Tax ID# of the new payee.
  3. A completed W-9 tax form (U.S. residents) or a W8-BEN form (Non-U.S. residents) for verification of your tax identification number or social security number. If the PAYEE CHANGE, is related to the death of the current payee, the following materials must be submitted in addition to the notarized copy of the form below which will represent a letter of direction in regards to the change you are requesting to our Legal and Business Affairs Department:
  1. A recently certified copy of the letters testamentary or;
  2. Necessary tax waivers, and;
  3. A notarized, written designation from the fiduciary concerned including the full name, address, and social security (or, if applicable, a Federal Tax ID#) of the appropriate payee.
  4. A completed W-9 tax form (U.S. residents) or a W8-BEN form (Non-U.S. residents) for verification of your tax identification number or social security number.
